LRMC offers appointments for sports physicals

The Landstuhl Regional Medical Center Department of Pediatrics will
offer its annual Sports Physicals Day by appointment only from 8 a.m.
to 3 p.m. Aug. 16 in the LRMC Heaton Auditorium.

The appointments are being conducted specifically to fulfill the school requirement for sports physicals.

Physicals will be provided for children ages 5 to 18. Now until Aug. 9, appointments will be made only for children currently enrolled at LRMC.

After Aug. 9, appointments can be made for enrolled and non-enrolled children. Parents may call the appointment line at 486-5762 or  06371-86-5762 to schedule. After your appointment has been scheduled, report to Heaton Auditorium inside LRMC at your scheduled appointment time. No walk-ins will be seen.

Items not covered at this appointment include medication refills, acute appointments/sick child visits, lab work or immunizations.

The Pre-Participation Physical Evaluation History Form must be completed prior to the appointment. The form can be obtained in the Pediatric Clinic, Family Medicine or by visiting www.aap.org/sections/sportsmedicine/PDFs/PPE-4-forms.pdf. Print and fill out Page 1. You can also ask for it to be e-mailed to you when you call.
